Informed Consent

I understand that tattooing is an invasive, permanent procedure that involves breaking the skin and introducing pigment. I have had the opportunity to ask questions and understand the nature, possible consequences and health risks of the procedure.

Risks & Possible Complications

Possible risks include, but are not limited to: pain, bleeding, bruising, swelling, infection, allergic or inflammatory reactions, scarring or keloids, pigment migration, colour change, uneven healing, loss of pigment, touch-up needs, and other unexpected skin reactions or complications. Healing varies by person and placement.

Health Disclosure & Impairment

I have disclosed relevant health information, allergies, medications, skin conditions and other factors that I believe may affect tattooing or healing. I understand the artist may postpone or refuse a procedure if proceeding appears unsafe. I confirm that I am not under the influence of alcohol or other judgement-altering substances at the time of the procedure.

Aftercare

I understand that proper aftercare is my responsibility. I agree to follow the aftercare instructions provided by the studio and to seek appropriate medical attention if I experience signs of infection or another concerning reaction.

Artwork, Placement & Changes

I have reviewed the agreed tattoo concept and placement. I understand that minor adjustments may be made for tattooing practicality, and that significant changes may require additional design time or a revised appointment. I understand tattooing is permanent and removal can be difficult, costly and incomplete.

Release & Assumption of Risk

To the extent permitted by applicable law, I acknowledge the inherent risks of tattooing and voluntarily assume those risks. I release the artist and Steampup Tattoos from claims arising from ordinary inherent risks of the procedure, except to the extent a release is prohibited by law or the claim results from conduct that cannot legally be excluded or limited.
